When measuring the gap between the cam and 'Cam follower' should the engine be hot or cold?
What is the exact procedure and calculation?

depends on the manufactures mantainance specs
but in general cold engine what I remember..
we would let them stay overnight and measure clearance in the morning

yes that is right it gets smaller when temp. is rising
the whole idea is to still have a small gap when the engine is hot (to make sure the valves are 100% closed when not engaged by the cam)
some manufatures give clearance specs for both cold and hot engine but if you only know the cold setting, it will be difficult to calculate the exact hot setting (you need to know material, diameter / mass of camshaft, percentage of volume increase related to temp etc.
I believe there is some generic cold to hot conversion rule, but I forgot what it is.....
